Home and Community-Based Services (HCBS) – Colorado DD & SLS Waivers

Our services are offered through Colorado’s Medicaid-funded Developmental Disabilities (DD) and Supported Living Services (SLS) waivers. We begin with a behavioral assessment that includes observation, interviews, and data collection to understand strengths, challenges, and goals. From this, we create a personalized behavior support plan that guides our sessions and ensures services meet the goals of clients and stakeholders.

Depending on the individual’s needs, sessions may include practicing coping skills, journaling or self-monitoring, role-playing social interactions, building independence with daily living skills (ADLs), or learning to safely access the community. In addition to direct client support, we provide training and coaching for caregivers, schools, and program staff so that strategies from the behavior plan are consistently applied across all settings. This collaborative approach promotes lasting growth, greater independence, and a higher quality of life.

Community Integration & Life Skills

Our ABA services are designed to help individuals build the skills needed to live, work, and participate fully in their communities. Our clinicians provide personalized support in areas such as self-care, daily living, social interaction, and employment readiness, always aligned with the individual’s personal goals, assessment and service plan.

We collaborate closely with caregivers, support staff, and community partners to ensure skills are practiced and reinforced across real-life settings. This approach promotes dignity, independence, and meaningful participation in the broader community, supporting each person in reaching their unique goals.
